Remedy offers a fully integrated, evidence-based chronic disease management service, delivered via a mixture of telephonic and in-home care models. The services are primarily targeted at improving patient care and service quality after patients are discharged from hospital. We aim to increase patient safety, independence and confidence in managing their condition at the same time as reducing the risk of returning to hospital.
