Remedy’s chronic disease management system (CDMS) is a sophisticated and scaleable chronic disease management software system which provides customisable care plans, risk management capabilities, scheduling and decision support tools. Being web based, it also has the potential for interfacing with our customers’ care providers, including GPs, hospitals, specialists and pathology laboratories.
The Coronary Artery Disease, Diabetes and Bone Health Programs use the CDMS as the primary system to support the program delivery.
