Coronary Artery disease – the facts

Coronary artery disease (CAD) is a condition in which fatty deposits (plaque) builds up inside the arteries of the heart, causing them to narrow and increase the risk of heart attack, stroke and other diseases of the vascular system. In some cases, plaque breaks off and blocks the blood flow, causing heart attack. Heart disease is the leading cause of death in Australia.*

Remedy Healthcare's Coronary Artery Disease Program has been designed to help patients self-manage their heart health risks and minimise heart attacks or hospitalisation.

What we do

The Coronary Artery Disease Program is a telephone based chronic disease management program available to members with coronary artery disease. The program is run by Accredited Practicing Dietitians and aims to increase your confidence and knowledge to manage cardiovascular risk factors and reach targets outlined by the Heart Foundation while working with your usual doctors.

The Coronary Artery Disease  Program includes education and support on many risk factors that can affect heart health such as diet, activity, medications, cholesterol and diabetes. Our aim is to help members live well and stay out of hospital.

The program is run in addition to your regular care by your GP and/ or specialist. It is an opportunity to gain additional support, education and skills to best manage you condition.
