HealthierMe is a phone-based health coaching program aimed at supporting your people with the lifestyle and behavioural changes to prevent or help cope with long-term health conditions.

Delivered by accredited and qualified clinicians, HealthierMe combines techniques known to improve health outcomes with a person-centred approach:

• Self-management support, e.g. understanding and managing medications, managing conditions like diabetes or osteoporosis.
• Behaviour change, e.g. getting active, avoiding injury, managing weight loss.
• Care coordination, e.g. navigating the health care system and linking to the services they need.

All HealthierMe programs are individually tailored to the client and designed to work alongside a person’s current treatment plan. Our HealthierMe coaches will work with your client over the phone using telehealth. This can be supplemented by Remedy Healthcare’s experienced in-home healthcare team to assist people from the comfort of their own home, where required.
